Inspired by her cousin Janine Lindemulder's highly successful porn career, Kelly Madison and her younger, graphic designer husband started an internet porn business. The business started as just a personal website, due to initial concerns that Madison had never posed nude before and that she was starting in the business at a much older-than-typical age. After the initial success of the website, the couple expanded into producing and filming pornographic DVDs starring other performers. Their company was called 413 Productions, after the street address of the warehouse they rented for filming. The PornFidelity web site and a matching DVD line grew out of some threesome scenes with the couple and women whom Madison brought home. The couple divided the business responsibilities: Ryan handled the production company, editing, photographing, web hosting, programming, advertising, and web traffic generation. Madison handled talent booking, arranging sets, company finances, and promotions.
Other ventures
Madison has launched a series of pornographic web sites. KellyMadison.com focuses on her public appearances. PornFidelity.com focuses on threesomes, swinging with her husband, and women that she brings home. KellyFind.com is a pornographic search engine, specializing in searches for specific types of performers. KellyCash.com is an affiliate program for the first two sites, replacing 413dollars.com in mid-2006. In 2015, Kelly directed a film Women of the Middle East. In 2020, multiple women accused Ryan Madison of sexual abuse and exceeding their boundaries in scenes for Kelly Madison Productions. They described being introduced to Kelly and other women in a pre-production house to create a false sense of security before being transported to the shoot house with Ryan where he would offer them alcohol or drugs. They accused him of engaging in rough sex without discussing it with them beforehand and asking to ejaculate inside them during the scene and doing so, sometimes without their consent.

Madison has also appeared in Scoremagazine, and she has written a column for Juggs magazine as well. On July 25, 2007, Madison and her husband were interviewed on the E! channel's Chelsea Lately television program. They discussed their marriage, their open relationship and pornographic career.
